2. PROCESSING OF DOCUMENTS
The
nurse candidate must submit the below documents required for Employer-based
immigration sponsorship to ESS. ESS can also assist the nurse with immigration
for their immediate family as well, waiting time is approximately 6 months
All applicants desiring employer-based immigration sponsorship for permanent
residence in the USA must provide the following documents for proper evaluation
by Enterprise Staffing Solutions

of CGFNS certificate, if any |

of certificates of completion of continuing education courses |

of credential evaluation report, if available |

- For
Philippine applicants, BCIS now requires certificates authenticated
by the National Statistics Office; We can submit your existing
local birth certificate, however, you may still be required to
present the NSO certificate in the future. Please apply for the
NSO certificate as soon as possible.

of marriage certificate, if applicable. |

Four
(4) color photos complying with the enclosed specifications –
if you are applying for a travel document, please submit six (6) photos
instead of 4. |

- If
you are not currently authorized to work in the U.S., you will
be required to submit an affidavit of support. The affidavit may
be executed by anyone such as a friend or relative. We will prepare
the affidavit form for your relative or friend.

you are currently authorized to work in the U.S., please submit
the following documents:

a.
Copy of W-2 forms for the last 3 years, if applicable.

b.
Original employment letter or in the alternative, copies of
your latest payroll statements for the last 4 pay periods.

of INS receipt notices for all applications to extend status. |

Visa
screen certificate, if available – if you have not yet obtained
one, please make sure you apply for the certificate as soon as possible. |
